Additional Information
This is a known limitation with PDF reports; if the template uses a font that is not recognized by the PDF report engine, the font is substituted. The solution is to embed the fonts in the .docx template before uploading the template to the ArcGIS Survey123 website. For more information, refer to the following link,

Workaround
Convert the .docx file to a pdf by saving the .docx file as a .pdf file in Microsoft Word.
