Setting the title of a form in Map Viewer displays the field name instead of the value of the selected attribute.

Workaround

Follow the workaround below to display the form correctly in ArcGIS Field Maps.

1. Return to the Configure form configuration window.
2. Additionally, add the field that is included in the title as an element in the form. This field appears twice on the form now, in the title, and as an element in the form.
3. Open ArcGIS Field Maps on a mobile device. Select an object and start editing. The form title displays the corresponding field value in the title, and also as an element in the form.
