Additional Information

This is expected behavior when having separate tabs open in a browser. Saving information in one tab does not automatically update the information in another tab. The reason the smart form is removed after saving the web map changes is that the first tab is still referencing the previous state of the web map before the smart form was configured. After having opened an existing web map in the new map viewer in one browser tab, and then creating a smart form through the Field Maps web application in another browser tab for that same map, before saving any changes to the web map through the new map viewer, that browser tab needs to be manually refreshed to ensure that tab is referencing the current version of that web map with those recent changes. In the upcoming release of the Field Maps Web application, the ability to open the web map directly within the same browser tab to make and save changes to the map is available, which should also help to avoid this issue from happening.
