Additional Information
The Add Join tool requires a unique table name and a unique field name to work properly. Upon joining the second time, this violates this uniqueness. A possible solution is to use %memory% data instead of the query layer, then use the Join Field tool.
Workaround
Use %memory% data instead of the query layer, then use the Join Field tool.
