Known Limit
After review by the development team, it has been determined that this issue is related to a known limitation with the software that lies outside of Esri's control. The issue's Additional Information section may contain further explanation.
Additional Information
For map and feature services published from ArcGIS Pro or ArcMap, the data returned by the map display and feature service data is equal. It is by design that the two services are combined in the table of contents of Map Viewer. This allows for an optimized high resolution map display, combined with the ability to edit features. The automatic combining of ArcGIS Pro and ArcMap based in the table of contents is by design. Please use the documented work around as necessary.
The behavior described above does not apply to GeoEvent based map and feature services, where the map may display an image that is a summarized or binned version of the actual data returned by the associated feature service
The unique GeoEvent use case is documented as:
BUG-000110632 GeoEvent based map and feature services that are related should not become combined together when both are added to the Map Viewer table of contents.
Workaround
Add the map service and related feature service into the same web map, create a copy of the map service, and then remove the original map service. Following that users should see that the copy of the map service is separate from the feature service in the table of contents.
