Only visible labels will convert to annotations. This does not work if a scale range needs to be set for all the labels in more than one class.

Workaround

5. Work around: After you have set up your labels the way you want them, then make sure there is a tiny over lap of scale so that all of the labels at one time are all on and drawn in the visible extent. The labels that are not drawn in ArcMap when you convert to Annotations, will not carry over to the Annotation layer. For example, one group of labels can be set for “do not show labels when zoomed in beyond 1:3,000”. While the other layer turns on when “Zoomed out beyond 1:299,999. This will not work for more than two classes.
