This issue is not reproducible at Survey123 version 3.14.Workaround
- Delete all cells with the text format under the name field in the StructuresBUG CSV.
- Update the survey form in ArcGIS Survey123 Connect, and publish the survey.
- Submit a record in the ArcGIS Survey123 field app (Desktop or mobile).
- Refresh the inbox folder and the note question is populated.
