Additional Information
Non-reproducible in the latest version of Survey123 for ArcGIS.
Workaround
Add the Enterprise Portal from the 'Add Portal' option in the Survey123 for ArcGIS field application:
- Click the settings wheel on the top-right corner of the app.
- Click 'Add Portal' and enter the URL.
- Click the hamburger menu > Sign In > select the Enterprise login. Sign in.
