Map extent set in a web map is not honored in web apps when using a vector basemap, unless the vector basemap uses a spatial reference with WKID 102100.

Workaround
In ArcGIS Web AppBuilder, after clicking ‘Use web map's default extent’, click ‘Use current map view’. Save and launch the web app.
