Layout toolbar not accessible if toolbar was not displayed in Layout View, while ArcMap is shut down without saving.

Additional Information

Thanks for reporting this issue, a few others have reported it as well and it is fixed in beta 2.

Workaround

Resetting the Normal template allows the Layout toolbar to be displayed again.This issue was reproducible in 9.4beta (2217) and 9.4 Weekly Build (2238) using WinXP.
