It is unclear whether a Web map needs to be shared with 'Everyone' if it is published as a Web Application using the Arcgis.com templates. Choosing to host it locally or at Arcgis.com results in different behavior.

CR222010 - you can get it working by setting up a proxy page then opening index.html and setting the proxyUrl value to their proxy page.Workaround
Share the map with 'Everyone' if the Web Mapping Application is hosted locally, or only share the map with a group and host the application at <a href="http://Arcgis.com" target="_blank">Arcgis.com</a>.Steps to Reproduce
