Survey123 Connect for ArcGIS and the Survey123 web designer are using different mechanism to compose the survey. In attempts to match the functionality, the user interface difference can be expected between Survey123 Connect for ArcGIS and the Survey123 web designer. The display of the single choice question looks different in the field app and the web form, and this is expected.
