In the ArcGIS Survey123 field app (Android), the shutter sound for image questions plays when taking a photo when the device's shutter sound setting is disabled.

This is the expected behavior. There is currently no setting in ArcGIS Survey123 for the camera shutter sound. Changing the setting in the default camera app of the device does not affect settings in the ArcGIS Survey123 camera, they are different apps and do not share settings. The same applies to flash, zoom, camera in use, and image size settings. These are independant apps and therefore do not share any settings.Workaround
Mute the device's volume.
