Additional Information

Confirmed user configuration issue. Use the webclient URL (this is used for older surveys). The best option is to grab the URL from the under the globe icon in Survey123 Connect after publishing. Example: https://survey123.arcgis.com/share/c45965e0a8f04e7ca4b09d244a6237ea?field:example=TEST
