Additional Information

As per the documentation a period (dot) is not supported in name column or choice list name column in xlsforms: https://doc.arcgis.com/en/survey123/desktop/create-surveys/quickreferencecreatesurveys.htm#ESRI_SECTION1_8ED1E0D010424885A425D6BEB893216A This is also clearly noted in the types sheet under the Special Characters section (row 170) in the templates and samples.
