In ArcGIS Pro ModelBuilder, using a .lyrx template file to define the output data symbology and labeling does not create the expected <LayerFile> tag in the metadata.

The behavior of storing the layer file symbology in an output dataset is working as designed for model tools. The model must have an output parameter that is not type derived, then the output dataset's metadata will include the layer file associated with the model parameters Symbology property. The models that were submitted by this defect BUG either did not have an output parameter, or had a derived output parameter.

Workaround

Use tools in the model that have a standard output parameter. Set the tool output as a model parameter, and assign the model parameter's Symbology property to an existing layer file. When the tool runs, the output dataset metadata will store the reference to the layer file. As a result, when the dataset is added to any ArcGIS Pro map, the symbology and other layer properties will match those of the specified layer file.
