In ArcGIS Monitor, an ArcGIS counter configured with an ArcGIS Server site which uses Integrated Windows Authentication (IWA) does not pass the credentials of the ArcGIS Monitor Server service account to the generateToken request as indicated in the token URL tooltip.

This version has reached mature support. Upgrade to the latest version of ArcGIS Monitor.Workaround
Use a valid set of credentials for web-tier authentication as the credentials since the service account credentials do not get passed to the generateToken request.
