In ArcGIS Server, when using the out-of-the-box print service to print the smallest scale of a cached ArcGIS Server map service with an A3 or A4 template, the output map produced contains the basemap but not the cached map service.

Additional Information
The request going to the print service has both minimum and maximum scales set for the cached map service layer. When zoomed in beyond the scale threshold, the layer does not draw. Clearing out the maximum scale for the cached map service resolves this issue.Steps to Reproduce
