Additional Information
This is expected behavior in ArcGIS Collector and ArcGIS Field Maps. In this particular situation, the attribute field used to configure the subtypes also has a coded-value domain applied to it in the main attribute table. As a result, when editing inside ArcGIS Collector or ArcGIS Field Maps, the coded-value domain is being utilized as opposed to the subtypes. As a result, the corresponding subtype configurations are not being applied correctly.
In this case and in general, the coded-value domain must be removed from the field definition in the main attribute table for the subtype field to function properly in ArcGIS Collector or ArcGIS Field Maps.
Workaround
After making the initial edit of the primary domain field, submit the change, and edit the point again to be presented with the proper subtype fields.
