In ArcGIS Experience Builder, the Filter and/or Query widget's 'Ask for values' (Unique) set input type fails when there is a Map Action enabled on a Map Server data source.

Additional Information
Duplicate of BUG-000140112. This is an issue with ArcGIS service, and is fixed in ArcGIS Online 11.3 in BUG-000140112. It is also a duplicate of BUG-000159496.Workaround
The current workaround is to remove the Map Action: Extent Changes > Framework > Filter data records on the Map Server data source.
