This issue is not reproducible in ArcGIS Experience Builder and ArcGIS Survey123 when using the proper "mode=" URL parameter for the desired workflow (i.e., "mode=edit" vs. "mode=new"). If this issue occurs, please reach out to Esri Support Services, as additional information is needed for further investigation.
