If a geopoint question in a survey uses the same name as the form_id, attempting to generate a report with any template fails and returns the error message, "An error occurred when checking the report template. Failed to parse ${geopoint_name} undefined".

Workaround

Change the name of the geopoint question and update the name of all other calculations in the form accordingly. Changing the form_id is not recommended as it may cause failure in republishing the survey or overwriting the existing feature layer.
