Fields in smart forms disappear after refreshing the ArcGIS Field Maps web application when the layer is created in ArcGIS Online using the 'Define your own layer' option.

This issue is no longer reproducible in the current version of ArcGIS Online. If the issue occurs, please contact Esri Support.Workaround
After refreshing the page, the field shows up under the Field list. From there, click the 'Add all' button to add the field and save it again.
