Choice lists in Survey123 are expected to display the label in the survey. Refer to https://doc.arcgis.com/en/survey123/desktop/create-surveys/xlsformessentials.htm#ESRI_SECTION1_5310617C66D341BC920C737C84501DF1 for more information in the 'Choices worksheet' section. However, the name column in the choices worksheet specifies the value persisted in ArcGIS (what is submitted to the data). The label column displays the answer choice as this is to be displayed on the form.

Workaround

Use any other layer type other than an SDE view layer.
