Bug
Database users created in ArcMap or ArcCatalog are able to connect to the SQL Server geodatabase from Insights for ArcGIS, while those created in SQL Server Management Studio are unable to do so.

Workaround
Grant db_datareader permissions to the SQL Server database user. Revoking the permission allows the user to continue connecting to SQL Server for a limited time.
