This issue is addressed in ArcGIS Pro 3.5. The reported issue is that the user-specified dimension arrow angle is used during feature creation, but changes direction once the edit is complete. Dimensions disregard the user-specified symbol angle and change the angle as necessary for inward- and outward-facing dimensions. This behavior is by design. The fix to this bug ensures that the dimension-controlled angle is maintained during feature construction and after the edit is complete, with no changes to the symbol angle.

Workaround

To create dimensions where the arrows point away from the extension lines, use an arrow marker symbol pointing to the left. Edit the Starting point symbol and Ending point symbol in the dimension style and either insert a shape from a font containing a left-facing arrowhead or insert a shape from an SVG file with an arrowhead pointing to the left.
