Additional Information
This behavior is no longer reproducible with the current ArcGIS Online version. For existing layer and map, add the custom symbol again and save the map. The image is displayed in Map Viewer and ArcGIS Dashboard.Workaround
Open the dashboard in ArcGIS Dashboards Classic.
