Attachments in a related table cannot be viewed in the Individual Response section on the Data tab in survey123.arcgis.com when the data is hosted in ArcGIS Server, Portal for ArcGIS and ArcGIS Online.

Additional Information

This is a known limitation of using ArcGIS Enterprise (Portal) and the Survey123 for ArcGIS website for printing individual reports or feature reports as keywords are not supported. Keywords are required for attachments to be able to link the attachment to the image question so that it can be displayed in the report.
