Duplicate of BUG-000150151: This is fixed in the 22.3.1 update for ArcGIS Field Maps on iOS and Android.Workaround
Alter the radio button choices under 'Features and attachment delivery' to force the Hosn to populate or add the following lines of JavaScript Object Notation (JSON) manually:
