Additional Information
The behaviour you are seeing on Android is actually expected and by design, it is not a bug.
Based on previous user feedback and requests, many users wanted to see the ~\ArcGIS\My Survey Attachments\ folder when browsing the gallery from within the app. Previously the gallery browse button only showed the images in the DCIM folder. If using the inbuilt camera in the app then the image taken can not be selected again as it is not saved to DCIM, but saved directly to ~\ArcGIS\My Survey Attachments\. Therefore the app now searches and displays images that are in the DCIM folder and subfolders as well as images that are in the ~\ArcGIS\My Survey Attachments\.
The consequence of this is that if you use the browse button to select a image from the DCIM folder which was taken with the device camera app, the image is renamed with a unique identifier and copied into the ~\ArcGIS\My Survey Attachments\ so it can be attached to the survey form. If you then browse and select the same image in DCIM folder and attach to another survey or the same survey but different question, it will copy the image again with a new unique identifier. This is by design to avoid the same image being attached to many surveys and then being deleted or renamed and that change applying to all surveys. Once you have done this a few times for the same image, the next time you browse the gallery, you will see the same image duplicated 2 or more times, but in fact each image being displayed is a different file name.
Workaround
This issue only appears in-memory it seems and does not save a duplicate image on the devices photo albums.
