An error occurs when generating a report from the Survey123 for ArcGIS website when the survey is created from an existing feature service with existing attachments.

Workaround

1. Navigate to the REST endpoint of the service and append a /1 for the Object ID to access the attachment info. The "Keywords" that are associated with the attachments can be seen.
2. In the survey created from the Feature Service, the name for the image question will be different than the keywords associated with the images.
3. Change the name of the image question in the XLSForm to the same name as the "Keywords" and republish the survey.
4. The connection between the attachments and the survey should be restored.

Steps to Reproduce
