ArcGIS Configurable Apps and the GeoForm app template have been deprecated and soon will be fully retired. It is recommended to use the Reporter template in ArcGIS Instant Apps or ArcGIS Survey123 moving forward in newer versions of ArcGIS Enterprise and ArcGIS Online.Steps to Reproduce
