Adding a system geoprocessing tool to any ArcMap toolbar via the Customize dialog box under the 'Geoprocessing Tools' component category does not persist the tool in a new blank session of ArcMap.

Additional Information

This is a known limitation. Geoprocessing tools added to a toolbar are only supported in customizations saved to a specific mxd. Customizations saved to the normal mxt do not save geoprocessing tools. This doc topic explains the concept of customizations saved to the normal template vs. those saved to a specific mxd. http://desktop.arcgis.com/en/arcmap/latest/get-started/customizing-the-ui/creating-a-new-toolbar.htm "When you create a custom toolbar—in ArcMap, for example—it will save with the current map document by default. If you want to save a toolbar in the Normal template so that all documents see it, uncheck the Create new toolbars and menus in document check box on the Options tab on the Customize dialog box. If you save it within the .mxd, it is only available when you use that .mxd file."
