This issue will not be addressed as there are suitable workarounds available: Please use ArcMap 10.5.1 and later, ArcGIS Pro, or TilePackage Kreator to create your Survey123 .tpks.Workaround
Workaround 1 :
- Create a custom scheme using the 'Generate Tile Cache Tiling Scheme' tool.
- Go to File > Share as > Tile package.
- Tile Format option > import the tile scheme here.
- Provide Summary and Tags > create the package.
Workaround 2:
- Upgrade to ArcMap 10.5 or ArcMap 10.6.
Workaround 3:
- Use ArcGIS Pro.
