Additional Information
This is an expected behaviour. Reverse geocoding in Survey123 for ArcGIS (field app and web) is a credit-based request which requires the supply of a valid token.
An explanation for this is added to the documentation: Reverse geocoding can only be used for a public survey if a proxy geocoder (with embedded token) has been configured by the survey author. The document also includes instructions (or link to existing instructions) on how to set this up.
Workaround
The reverse geocoding question executes for a user without an ArcGIS Online account taking a publicly-shared survey when the survey is opened in a web browser.
