Additional Information

An incorrect template is being used. When selecting a template with grouping, a field must be selected to group by. If no field is included, the group header is not created. This results in a report that does not include field headers. If recreating the report using the Attribute List template (and not the one that includes grouping), the result includes field headers. This Adobe's behavior cannot be determined; however, it probably recognizes this report differently.
