When adding a second machine to Portal for ArcGIS that is utilizing Insights for ArcGIS, the addition of the second machine causes the Insights for ArcGIS icon to disappear from the Apps page. The documentation should state that re-indexing is necessary after joining second machine to Portal HA.

Workaround

Re-indexing restores the icon back to the Apps page.
