The issue is currently not reproducible and may have been resolved along with other fixes.Workaround
Take a photo using an app other than the mobile's own camera, then add it to the survey from the gallery. It is important to take photos using an external app; if the photo is taken with the mobile's own camera and is added from the gallery, the workaround does not work.
