This issue is not reproducible at Survey123 version 3.14.

Workaround

  1. Delete all cells with the text format under the name field in the StructuresBUG CSV.
    1. Capture_03009557_4.JPG
  2. Update the survey form in ArcGIS Survey123 Connect, and publish the survey.
  3. Submit a record in the ArcGIS Survey123 field app (Desktop or mobile).
  4. Refresh the inbox folder and the note question is populated.
