In Web AppBuilder for ArcGIS, enabling Use proxy for subscriber content access after the app is first created gives the error 'Failed to create proxy for <service URL>'.

Workaround

Refresh the app after it is first created prior to enabling the option to Use proxyOR Create the proxy for the service through Content and then add the item with stored credentials to the web map, prior to creating the Web AppBuilder for ArcGIS application.
