In Survey123 web designer. regardless of the amount of images that are added within the survey description from file prior to the initial publish of the survey, after publishing, if an additional image is added to the survey description form file, the survey will be unable to publish. If the additional image is removed after the second publishing attempt, the survey continues to fail to publish. 

Workaround

Workaround #1:

Edit Web Designed Survey in Survey123 > Edit the Description Text question in the Hint column, remove the HTML portion that has the image listed in the error (or provide the name of the image that's in the media folder to replace it) > Publish from Survey123 Connect > Survey works and publishes as expected.

Workaround #2 (Intermittently successful):

Remove all of the images > Publish > Add all images desired within the description > Publish >The result will have no error and published successfully.
