If the Damage Assessment Solution's Individual Assistance Survey is published without any changes, it will indicate that the current feature service is incompatible with the survey and will be removed from ArcGIS Online.

Workaround

1. Open ArcGIS Survey123 Connect and download the Individual Assistance Survey.
2. Open the XLSForm.
3. At the bottom of the spreadsheet, click the Settings tab.
4. Change the form_id from “Individual Assistance v4.0” to “Damage Reports”.
5. Save the XLSForm.
6. In ArcGIS Online, browse the Indivisual Assistance Survey feature layer view and open the item details.
7. Under Layers, click Individual Assistance Survey (v4.0).
8. In the blue header, change the name from “Individual Assistance v4.0” to “Damage Reports”.
9. Click Save.
10. Open ArcGIS Survey123 Connect and make changes as needed.
