This issue is non-reproducible with the latest ArcGIS Field Maps version. For the Field Maps mobile app, the recommended approach is to use Field Maps Designer to configure the form. The Level_ID field is typically hidden because it is automatically populated based on the floor filter selection in the mobile app and there is no need for an Arcade expression. If this issue persists, reach out to Esri Support Services as additional information is needed for further investigation.
