Created templates within Survey 123 for ArcGIS are giving a repeat of fields rather than creating new fields in each repeat. Although different field names in the smart form are being used, using those field names within the template produces the same label for the fields within the smart form. When the labels within the form are changed and re-uploaded, the survey is getting the correct fields in the print template.
