Adding an image to the survey header will cause the survey not to load from the survey card in ArcGIS Hub.

Workaround

Use the Embed option in the survey card or add the survey URL to the ArcGIS Hub site instead, so the user is taken directly to the survey in ArcGIS Survey123.
